● Modulates cell adhesion

Terminology

● Also called integrin associated protein (IAP), antigenic surface determinant protein OA3

Physiology
=========================================================================

● Encodes membrane protein which mediates increase in intracellular calcium after cell adhesion to extracellular matrix
● Modulates cell adhesion by altering the avidity of integrin binding and by interacting with thrombospondin, the extracellular matrix protein (J Cell Physiol 2005;205:182)
● The immune system recognizes invaders as foreign because they express determinants that are absent on host cells or because they lack markers of self that are normally present; CD47 is a marker of self that binds to SIRPalpha inhibitor receptor on macrophages and dendritic cells to inhibit phagocytosis, and limits destruction of host cells in autoimmune diseases (Leuk Lymphoma 2004;45:1319, Transfus Clin Biol 2006;13:39)
● May have role in giant cell creation from macrophage fusion (J Biol Chem 2000;275:37984)
● Plays an important role in CNS memory formation and synaptic plasticity in the hippocampus

Clinical features

● Expression on tumor cells may block phagocytosis by macrophages; anti-CD47 antibodies are being studied as combination treatment with rituximab (Cell 2010;142:699)
● CD47 on tumor cells is accompanied by calreticulin, a pro-phagocytic marker (Sci Transl Med 2010;2:63ra94)
● Reduced CD47 expression on monocytes is associated with burn severity and the occurrence of multiple organ dysfunction syndrome/MODS in burn patients (Burns 2009;35:937)
● CD47 / thrombospondin-1 have a role in cardiovascular pathology through their interaction with nitric oxide (Arterioscler Thromb Vasc Biol 2008;28:615)
● Helps maintain mechanical properties of RBC membrane (Leuk Lymphoma 2004;45:1319)

Uses for pathologists

● Measure storage related changes in red blood cells (Transfusion 2005;45:1496)
● Differentiate T-acute lymphoblastic leukemia (CD47+) from T-lymphoblastic lymphoma (CD47-, Pediatr Blood Cancer 2006;47:130)

● All hematopoietic cells, brain, endothelial cells, epithelial cells, fibroblasts, mesenchymal cells

Positive stains - disease

● Acute lymphoblastic leukemia (Cancer Res 2010 Dec 21 [Epub ahead of print]), acute myelogenous leukemia (Cell 2009;138:286), bladder carcinoma, non-Hodgkin's lymphoma, ovarian tumors
